2022 Formulary (List of Covered Drugs)
A formulary is an entire list of Part D drugs covered by CarePartners of Connecticut PPO.
PLEASE READ: THIS SEARCH TOOL CONTAINS INFORMATION ABOUT THE DRUGS WE COVER IN THIS PLAN
Please use our search tool to make sure that it still contains the drugs you take. While the formulary may change throughout the year, in general, if you are taking a drug on our formulary that was covered at the beginning of the year, we will not discontinue or reduce coverage of the drug during the year. If we remove a drug from our formulary, add any restrictions, or move a drug to a higher cost-sharing tier, we must notify affected members of the change at least 60 days before the change becomes effective.
