Important Message About What You Pay for Insulin For a one-month supply, you pay whichever is lesser of the cost - $0 for Tier 1, $20 for Tier 2 and won’t pay more than $35, for Tiers 3, 4, 5, -or 25% coinsurance of maximum fair price or negotiated price of each insulin product on Tiers 1 through 5 covered by our plan. You do not pay deductible on insulin products.
